What are the differences between the home care packages?
The Hospital in the Home service provides you with high-level care in your own home so that you don't have to stay in hospital. People under 65 who have a disability or a condition that restricts day-to-day living may be eligible for support through the Community Care program. “Knowing the benefits of HiTH to both patients and the health service, WBHHS set a goal to support 1.1% of admitted patients through our HiTH model of care. We have exceeded this goal, with over 4% of admitted patients receiving monitoring and treatment via HiTH. Premier Nursing Services can offer complimentary assessment and make recommendations as to the type of support needed.
